The ears are sensitive to sudden changes in air pressure, such as during takeoff and landing, scuba diving, or fast elevator rides. When the eustachian tube can't equalize pressure quickly enough, it causes fullness, pain, echoing, or ringing in the ear. Simple maneuvers like swallowing, yawning, or gently equalizing pressure usually relieve it, and avoiding flying or diving with a cold or congestion helps prevent it in the first place.
The middle ear is an air-filled space connected to the back of the throat by the eustachian tube, which normally opens briefly with swallowing to keep pressure equal on both sides of the eardrum. When outside air pressure changes quickly — as in an airplane cabin, during a dive, or in a fast elevator — and the tube can't keep pace, the eardrum is pushed or pulled, causing pain, fullness, and muffled or echoing hearing. Swallowing, yawning, chewing gum, or gently pinching the nose and exhaling against closed lips (the Valsalva maneuver) all help open the tube and equalize pressure. Congestion from a cold or allergies narrows the tube and makes barotrauma much more likely, which is why flying or diving while congested is best avoided.
