What's in it

The B-complex group includes thiamine (B1), riboflavin (B2), niacin (B3), pyridoxine (B6), folate (B9), and (B12), each a distinct water-soluble compound with its own metabolic role rather than a single unified substance.

Mechanism of action

Most B vitamins function as precursors to enzyme cofactors -- thiamine forms thiamine pyrophosphate and riboflavin and niacin form FAD and NAD, all central to mitochondrial energy production in the Krebs cycle. Folate, B12, and B6 work together in the one-carbon cycle, converting the amino acid back to methionine, a reaction also required for DNA synthesis and maintenance around nerve fibers. B6 additionally serves as a cofactor for decarboxylase enzymes that synthesize neurotransmitters such as serotonin and GABA. B12 deficiency in particular can impair myelin formation, which is why its depletion is associated with neurological rather than purely metabolic symptoms.

Current pharmacological applications

Deficiency states for individual B vitamins are well characterized clinically -- thiamine deficiency causing beriberi, B12 deficiency causing pernicious anemia and neuropathy -- and treatment of documented deficiency is well supported. Supplementation of a combined B-complex in non-deficient individuals has been studied for fatigue, mood, and cognitive performance with mixed and generally modest results, and large trials lowering with B vitamins have not consistently reduced cardiovascular events despite the biologically plausible mechanism.
